What is an effective Safety Culture? How do we know what we've got? How do we know what to change? How do we introduce & consolidate that change?

Safety Management is an essential element of a good Safety Culture, but it has developed the perception of being a black art. The fundamental question "how do occurrences happen and what do we do about it" needs to be answered in sensible, practical means. Quite often complacency or confusion sets in as a result of multiple definitions of safety culture and safety management which lead to expectation and support of existing safety behaviours. "Buying in" to the process is essential - participation increases that ownership. Early interventions are effective and, of course, managers must 'walk the walk'.
